My name is Dr. Stephanie Chambers DNP, FNP-BC and I am honored to partner with you on your health journey. With over 17 years in healthcare, it has been my passion to provide holistic and traditional services to individuals and families. Chambers Family Health NP P.C., also referred to as Your Door to Health & Wellness was established November 19, 2020.
Your Door to Health and Wellness services include health promotion, disease prevention, health maintenance, and chronic disease management services for adults 21 years and older in the Bronx, Manhattan, and Westchester county. The goal is to provide intensive holistic and traditional services, with a multidisciplinary team, to address the 8 dimensions of health and wellness, which are spiritual, social, emotional, intellectual, environmental, physical, occupational, and financial. An assessment of each dimension will enable the team to better meet your healthcare needs.
Services provided include:
Healthcare services for adults 21 years and older. Referrals are also provided as needed for more serious conditions.
Disease such as heart failure, diabetes, hypertension, asthma. Our team of specialists can assist in managing these conditions with group and individual medical visits.
Follow up appointments and urgent care visit are available to all patient after an initial in person health exam.
Personal trainers and nutrition guidance. A free complimentary consultation and one training session are available after an initial in person annual health exam.
Reiki, reflexology, yoga, aromatherapy/essential oil consultation, mindfulness and meditation, Red/near Infrared Light Therapy.
Serve as liaisons between social services and the community to provide social support, community outreach, education, and advocacy services, as needed i.e. Housing, meals, and transportation support.
Spiritual counseling.
